The Maryland Department of Health has new medication forms as of 2019. Please print and fill out the appropriate form depending on your child’s medication needs. There are forms for allergies, asthma, or seizures; for all other medication or over-the-counter meds, please fill out the Medication Administration Authorization form below. Physician and parent signatures are required for all forms.
Please bring forms and medication in their original packaging on your child’s first day of camp.
